This system transformation grant annual progress report is learning oriented and seeks to:

• Assess and report on the performance progress and achievements of the grant for the reporting period, to inform improvements in the implementation of the grant so as to ensure that its intended aims are fulfilled.
• Share lessons to improve the grant activities and priorities.
• Ensure accountability and transparency of the grant and its commitments.

This template should be used for system transformation grants that employ a project-type modality (stand-alone or cofinanced). For grants that use a sector-pooled modality, use this reporting template.
